    Description

    Manor Homecare are delighted to announce as we continue to grow we are looking for a care co-ordinator in North Dublin to join our team.

    General duties

    • Reporting to the Care Co-Ordinator lead, Assistant Manager and Clinical lead
    • It is the responsibility of the Care Coordinator to assist the Registered Manager to organise the day to day running of the service provision through the policies, training, supervision and guidance provided by the Company.
    • It is the responsibility of the Care-Coordinator to support the Registered Manager in achieving the aims and objectives of the Company and achieving the level of quality in the service provided to Service Users in accordance with the Company's Quality Assurance Policy.
    • To assist the Registered Manager in marketing the services provided by the Company and liaising with commissioners, prospective Service Users, health professionals and other agencies connected with training and development of the service.
    • It is the responsibility of the Care coordinators to support service users and healthcare workers by completing their rota to their preference in an appropriate and efficient way.
    • It is the responsibility of the care coordinator to monitor roster queries on a daily basis vis phone and e-mail
    • To ensure communication is prioritised between clients and other staff members To treat clients and staff with respect and deal with concerns in a courteous manner Duties and Responsibilities
    • To be responsible to the Registered Manager for the day-to-day service provision to the Service Users by producing detailed work schedules for Health Care Workers and providing both written and verbal instructions/guidance to them.
    • To be conversant with the HSE requirements and legislation governing the service. To learn new skills, further develop and refresh on existing skills in preparedness for the regulation of Domiciliary Care Services.
    • To be aware of the quality assurance policy of the Company in the provision of a quality service to the Service Users. To always represent the Company in a professional manner, on the telephone, face to face or in written communication. To ensure that telephones are answered promptly, and people are spoken to in a polite and respectful manner. To maintain confidentiality and carry out the Company's Confidentiality Policy.
    • To report to Team lead for any issues regarding the legislative requirements concerning the services provided and the Staff providing them.
    • To report to the Registered Manager any financial issues that have an impact on the overall budget.
    • To acknowledge and be responsible for all staff and client concerns that come though whilst appropriately managing concerns to avoid further complaints
    • To maintain accurate records and document all correspondence on One Touch to clients and staff
    • To be at least two weeks ahead on rosters
    • To send out rosters from one touch weekly and to always notify clients and carers of any changes one the final roster is issued
    • To participate in weekly meetings with the Team lead and management meetings as required Health and Safety
    • To be familiar with the Company's obligations under the Safety, Health and Welfare Act and other Health and Safety Regulations in the Company's Health and Safety Policies.
    • To keep the Registered Manager informed of the outcomes and issues that are identified concerning all Health and Safety issues. Quality Control
    • To assist the Registered Manager to implement the monitoring and quality control process under the Company's Quality Monitoring Policy.
    • To be familiar with and to implement the Companies policies and procedures and all legislative and regulatory requirements relating to the activities of the Company.
    • Ensure that telephones are answered promptly, and that people are spoken to in a polite and respectful manner.
    • To be aware of the Quality Assurance Policy of the Company in the provision of a quality service to the Service Users. General To assist the Registered Manager in the aim of the Company providing a quality service by a team of trustworthy, well trained and reliable Health Care Workers.
    • To support the Assistant Manager as required or when directed by the Registered Manager, or when the Registered Manager is out of the office.
    • Participate in the "Out of Hours" call rota. This will include every other weekend. To carry out any other tasks required by the Registered Manager that are commensurate with the level of responsibility.
    • You may be required to undertake Care Staffs spot checks, carry out quality monitoring assessments, provide services within the community should the need arise. Job Specification Essential: Experience in care provision at a level that reflects the responsibilities of the post applied for Good literacy and numeracy skills Good presentation skills Computer literate Understanding of legislation concerned with care provision Administrative experience Full driving licence Desirable: QQI Level 5 or working towards Professional qualification Experience as supervisor or senior carer care worker or other role within a care providing organisation.
