- Work within the framework of the overall aims and objectives of Focus Ireland and use the model of care devised for the project.
- Work as part of a team in delivering the service to the young people.
- Draw up suitable care plans to prepare and support each resident for independent living
- Support young people holistically, paying attention to their practical self-care needs, health, budgeting, and domestic skills.
- Play a part in promoting and building the confidence and self-esteem of young people leaving care
- Advocate on behalf of residents for housing, social work, social welfare, education, training and legal rights.
- Liaise with and make referrals to relevant outside agencies.
- Provide individual support and keyworking to residents of the project
- Assist in the facilitation of group work sessions for residents of the project.
- To be vigilant to any Health, Safety and Welfare risks in the workplace and bring any concerns to the attention of your line manager or Health & Safety Representative
- Attend Team, Divisional and Agency meetings as required
- Maintain accurate case records and statistics
- Liaise with and inform the Project Leader, on a regular basis, of issues arising from work with individual cases
- Engage in PIE and supervision sessions on a regular basis with the Manager.
- Participate in relevant training and development courses and in project evaluation
- Implement the existing policies and procedures of the project and of Focus Ireland and adhere to the Standards of Customer service.
- Assist in the implementation of the settlement policies and strategies of Focus Ireland
- Work in close liaison with and co-operate with other Focus Ireland projects.
- In consultation, to undertake other duties and responsibilities as may be assigned by the Project Manager, Section Manager or Divisional Head.
- Social Care or Equivalent Qualification at degree level.
- Experience working in a social care setting at least 2 years.
- Experience of key-working clients or individual case-work.
- Full clean driver's license and access to own car.
- Experience of managing challenging behaviour.
- Good understanding of the issue affecting young people leaving care and in aftercare.
- Flexibility.
- Understanding of Child Protection.
- Experience of inter-agency working.
- Proficiency with Microsoft Office Suite.
